2015-16 - Email List Subscriptions and Band Directory Form

If you submitted this form before or were signed up by someone else and you need to make changes, don’t fill it out again; use the private link in your confirmation email to view and EDIT your saved information! More than one child in band? Please submit this form once for each child so all students are listed in the Directory! Even though it’s a bit of a pain, include all the info for each student, BUT subscribe to email lists relevant to that child, so everything will be good if one child drops out or graduates and is removed from the database.

Only 1 parent? Just enter “none” for the other’s name.

Staff members - Put your information in the Parent 1 section and use the Parent 1 Email Subscription section. For other required fields, enter “none.”

Alumni & Friends - Put your information in the Parent 1 section, even if you were a student. Couples can enter names like “Tom & Mary” if you have one email addresses, or submit the form separately for each individual. For student name and other required fields, enter “none.” If you don’t mind, we’d appreciate your mailing address in the student section, and your your connection to BHSN Bands in the Note, eg, your children’s names and when they, or you, graduated from BHSN!

Instruments	Enter the instruments this student plays in school bands. Don't get too specific - eg, 'sax, percussion' is fine.
Directory Information If there is something you do NOT want to share with other band families in the Directory, PUT AN "X " IN FRONT OF YOUR ENTRY.
